The clinical observation of Ovarian Tumors

Abstract
A clinical survey was made on a series of 201 Ovarian Tumor cases, admitted, operated upon and confirmed with postoperative histopathological study at the Department of Obstetrics and Gynecology, Choong-Nam medical center during a period of 5 years and a month from January,1966 to February, 1971.
The results obtained are as follows:
1. The incidence of ovarian tumor during this period was 12.3% of the total operated cases in, the Department.
2. The incidence of benign tumors was 88.6% of all ovarian tumors and that of malignant tumor was 11.4%.
3. Mean age of benign Ovarian tumor was 35.0 years and that of malignant one was 40.6 years, showing 5 years difference in between.
4. As to chief complaints in all ovarian tumors, lower abdominal pain was noticed in 52.7% of cases and lower abdominal mass in 48.2%. In benign ovarian tumors, lower abdominal pain was noticed in 53.4% and in malignant ovarian tumors, lower abdominal mass in 65.2% of the cases.
5. Physical findings were as follows:
Ascites was observed in 16.4% of all ovarian tumors, 11.2% of benign tumors and 56.5% of malignant tumors. Cachexia was noticed in 2.9% of all ovarian tumors, 1.1% of benign tumors and 17.3%of malignant tumors. Adhesion with the adjacent organ was found in 27.8% of all ovarian tumors, in 23.0% of benign tumors and in 65.2% of malignant tumors. Combination with pregnancy was observed in 3.9% of all ovarian tumors, 3.9%, of benign tumor and 4.6% of malignant tumors.
6. 27.3% of all ovarian tumors were from nullipara, but the fact that increasing parity parallels increasing incidence of malignant tumor corresponds with the increasing tendency of malignant tumor in older age.
7. Primary sterility was noted in 10.4% of all ovarian tumors and secondary sterility in 7.9% of the cases.
8. 63.1% of all ovarian tumors in this period was most frequently occurred in reproductive period.
9. Menarch began at 16 years of age in 29.8% of the cases.
10. Menstruation was regular in 69.6/, irregular in 30.4%.
In benign tumors, menstrual cycle was predominantly regular, but in malignant tumor it was predominantly irregular.
11. In 36.7% of all case ovarian tumors was noted in¢¥ the left ovary, 45.0% in the right, and 18.3% bilateral in nature. It was striking that the greater occurrence of tumors was observed in the right ovary for both benign and malignant tumors.
12. Among the benign ovarian tumors, dermoid cysts were bilateral in 18.9% pseudomucinous cystadenoma in 17.0%, serous cystadenoma in 4.1%.
13. As to common size of all ovarian tumors, tumor with the size of an adult fist found in 24.3% of all cases, while one with the size of a child head was noted in 28.5%, of all malignant cases.
14. The incidence of pedicle torsion was 18.0% of all cases, 33.3% in fibroma of ovary, 25.5% in pseudomucinous cystadenoma, 18.7% in serous cystadenoma. 14.4% in dermoid cyst.
15. Pedicle tortion of the ovarian tumor occurred most frequently with the adult first-sized tumors in 50%.
It was found that there- was a tendency between the tumor size and tortion side, that is larger tumors have a tendency to the left side and smaller tumors to the right side.
16. Concerning hemoglobin content level, the value of 10 gm% or less was noted in 55.8% of all tumors.
17. In the benign tumors the incidence of dermoid cyst, was 38.7%, serous cystadenoma 26.9%, and pseudomucinous cystadenoma 26.4%, while in the malignant ones papillary serous cystadenocarcinoma 26.0%, and malignant teratoma 17.3%.
